Write a re-evaluation
RehabAlpha is still under active development. It is not yet HIPAA compliant and should only be used with dummy data.
Use a re-evaluation to document a reassessment of the patient's condition and treatment needs. A re-evaluation can continue the current treatment plan or establish a new plan.
Before you start
Review:
- the current plan of care
- patient progress toward goals
- remaining deficits
- medical necessity for continued skilled therapy
- whether the current treatment plan still reflects the patient's needs
- the proposed frequency and duration if you plan to establish a new treatment plan
Complete the re-evaluation
- Open the patient's therapy case.
- Click Re-evaluations.
- Click New re-evaluation.
- Enter the re-evaluation date and complete the clinical documentation.
- Under Treatment plan, choose one of the following:
- Continue current plan to keep the active treatment plan, including its original end date, frequency, duration, and anticipated interventions.
- Create a new plan to enter a new start date, frequency, duration, and anticipated interventions.
- Review the treatment-plan summary or enter the new plan details, including Anticipated treatment interventions.
- Save the re-evaluation.
When you choose Create a new plan for PT or OT, Anticipated treatment interventions includes G0283. Select it when that service is part of the new treatment plan.
The service-code picker offers HCPCS codes valid on the re-evaluation date. Anticipated treatment interventions offers codes valid at any point during the new plan. Selected codes stay visible when dates change. If a service code is no longer valid on the re-evaluation date, the signature checklist asks you to replace or remove it before you sign.
Continue the current treatment plan
Choose Continue current plan when the re-evaluation supports continued care under the existing plan. This can be useful when a therapist needs to perform and bill a re-evaluation using the appropriate re-evaluation HCPCS code without changing the patient's treatment frequency, duration, or anticipated interventions.
This option is available only when a treatment plan covers the re-evaluation date. Continuing a plan does not restart or extend it. The plan keeps its original end date.
After saving
Route the re-evaluation for signature if required by your organization or payor workflow.
You should know
- A new plan's Cert start date must be on or after the re-evaluation date. Continuing a plan keeps the original certification dates.
- Every supplied medical or treatment diagnosis Onset date must be on or after the patient's date of birth, when known, and no later than either the re-evaluation date or today in the document's time zone. Secondary medical diagnosis onset dates remain optional.
- Changing the service date does not overwrite diagnosis onset dates. Review any date errors before saving.
- You can save a future re-evaluation for planning, but cannot sign it before its service date.
- Evaluation weekday always follows the initial evaluation. A re-evaluation does not start a different therapy week.
